Know Your BenefitsPlease understand that our office has many patients with many different insurance policies. It may be surprising that even a single insurance carrier can have hundreds of different coverage plans available. Please note that health insurance does not always cover all your health care cost. Most plans require co-payments, co-insurance percentages, and deductibles, which must be paid by the patient at the time of visit. It is important for you to become familiar with your insurance policy and understand what services your policy covers. This will allow you to receive the maximum benefits of your specific insurance plan.

Participating ProvidersPlease make sure that our physicians participate in your specific insurance
plan. You can verify this by either checking your insurance plan provider
directory or by calling your carrier directly.
Similarly, if we need to refer you to a specialist, please refer to your insurance plan listing of participating providers to ensure that the specialist is in your plan network prior to making an appointment.

Insurance CardWe ask that you present your insurance card at each visit to our office.
We require a copy of your insurance card on file in order to file claims.
If you do not have your insurance card with you, we will ask that you
pay for services until that information is provided.
Payment at Time of ServicePayment in cash, check or major credit card is expected at the time
of service, and all co-payments, co-insurance, and deductibles will
be collected. Please be advised that there may be services that your
insurance company will not cover (new immunizations, flu shots, hearing
and vision screening, etc). We request full payment for these services
at the time rendered.

Insurance Claim FilingAs a courtesy to our patients with insurance coverage, our staff will
file your claim if you provide the correct information. Although an
insurance claim is filed on the patient’s behalf, the financial responsibility
for our services still remains with the patient and not their insurance
company. If we do not receive payment within 60 days, it becomes the
responsibility of the patient to contact the insurance company to expedite
claims processing.

Financial DifficultyWe understand that many patients have insurance plans with high deductibles.
If, at any time, medical care becomes a financial difficulty please
let us know. We are always willing to help you with payment plans or
other arrangements.
